Dear Collins Families:

Thank you for notifying the school if your child will be absent or delayed from school. If your child will be absent from school due to illness, please note the symptoms in the reason section. If your child will be delayed coming to school, please use this form as well as state the reason they will be late. This applies in Distance Learning as well.

to the school 24 hours in advance with support documentation where applicable.
