Completion of this form is only necessary if you would like to request a change to your student's current learning option.

- The deadline to request a change for the 2nd six-weeks grading period is September 25. These requests will be processed automatically. Parents will be contacted by campuses with pertinent information prior to the start of the six-weeks.

- Requests to change learning options at times other than the end of a grading period will be considered by campus-administration on a case-by-case basis.

- Please enter only one student per form. To enter learning requests for additional students, please submit this form and use the link corresponding with each student's campus to submit additional requests.
