Dear Students and families,

Please use the form below to report incidents that you've either experienced or witnessed. This form is monitored during school hours which are Monday-Friday 7:00AM-3:30PM.  If this is an emergency that requires immediate attention please call 911.  To report a possible crime, please call the local police for assistance.  If in need of psychological services, please call 211.

Any incident that negatively targets, intimidates, or threatens an individual or group due to race, ethnicity, ancestry, national origin, religion, gender, sexual orientation, gender identity or expression, age, physical, mental, and intellectual disabilities, as well as past/present history of mental disorders. Acts of harassments, bullying and/or discrimination include but are not limited to, verbal/written comments, graffiti, on-line posts, images that harass or intimidate individuals or groups. Reporting incidents helps to ensure that members of our community receive necessary supportive resources. These incidents may also indicate a need for education in our community in order to remedy harm done, and to provide opportunities for reflection and growth. Such educational efforts are not part of the investigation process and are not intended to be punitive.

When a report of an act of bias is submitted, the incident will be reviewed by members of the school administration (Dr. DeLoreto, Mr. Tanner, Mrs. Paruolo) and also shared with appropriate school support personnel (school counselor, social worker).   Each report is reviewed individually and managed on a case-by-case basis.  The bias response protocol is not a policy but a means by which E.O. Smith can provide support.  Our staff will review the report and take actions as appropriate.
The actions include:
identify and provide support to impacted parties
review incident to determine possible violations of the law and/or school rules
determine action steps to educate the impacted community and work to repair harm

When an incident is reported, a school administrator will be assigned to your case.  Frequently this includes interviews with impacted parties. Often during the beginning of an investigation, fact-finding is occurring which is not shared publicly.  The complainant will also be offered the opportunity to have a trusted E.O. Smith staff member to be present. Interim measures as appropriate may be offered and can include, no contact orders, changes to school schedule, academic and social-emotional support.  When an incident occurs off campus and is under investigation, E.O. Smith will work with local law enforcement agencies.

Please be aware that intentionally creating a false report is a violation of school rules and may result in disciplinary action.
