LDS Missionary Health Survey
As a life-long Mormon and former reporter for the Salt Lake Tribune, I am now a freelance journalist working on a story for submission to the Associated Press. The story I'm working on explores reports that certain remote missionary assignments have posed medical challenges, specifically gastrointestinal illness. I am trying to determine to what extent this is happening—that is, whether other missionaries have experienced anything similar. If you have anything that you think relevant, perhaps you could fill out the form below. I would appreciate hearing from you. Thanks.

When and where did you serve your mission? *
While serving your mission, did you experience gastrointestinal symptoms or distress beyond what you would expect from a stomach flu? If yes, please describe your symptoms and how long they lasted.
While serving, were you issued any form of protective gear such as water filters, filtration water bottles, disinfectants, etc? If yes, please describe what you received.
If you became ill while serving, did you seek medical care in the field? If yes, please tell us about your experience?
I will not use your information without your consent. If you are willing to discuss/share/talk about your experience in the mission field, how may I contact you?
If your experience is relevant to the story, would you be willing to be quoted by name in the story? *
