Please thoroughly read and tick the following guidelines before filling out this form. *

All student records are considered legal documents and are therefore processed with utmost care and diligence.

Records are usually released after the year-end ceremonies unless other feasible arrangement has been made.

All records require a certain fee except for SF9/Form 138/Progress Report when requested for the first time.

You can pay by fund transfer, bank deposit, or G-Cash.

No documents will be processed without proof of payment. Upload your proof of payment to complete the processing of your request.

If my record will be delivered, shipping charges are on me PLUS a transportation fee of P100.00 which is also deposited ahead and included in the proof of payment.

Form 137 is a school-to-school transaction and therefore can be released only upon the request of the receiving school usually during the 1st Quarter. Parents who request it ahead will have to bring a request form from the receiving school authorizing them to hand carry the form and pay the processing fee of P150.
