PLEASE SUBMIT ONE REGISTRATION FORM PER CHILD

This is the initial registration form for CCRC After School Care. Registration fee ($40 per child) and weekly payment ($40 per child/ week)  is required to be paid to continue participation in the program.

YOU WILL RECEIVE AN EMAIL WITH ADDITIONAL FORMS AS WELL AS PAYMENT INFORMATION ONCE THIS FORM IS SUBMITTED. THIS INFORMATION AND PAYMENT MUST BE SUBMITTED PRIOR TO YOUR CHILD'S SPOT BEING RESERVED.  Any unpaid accounts will be moved to the waiting list.
