Tzu Chi volunteers are organizing this program in their own personal capacity to promote plant-based diet to the public in general with no fees charged whatsoever. Any arrangement or transaction with any parties in relation or incidental to this program including but not limited with food providers and clinics are to be dealt directly between the participant with such party with no intervention, interference or management from Tzu Chi volunteers.

It is also hereby expressly stipulate and made known to the participant that this program is neither relevant with Tzu Chi nor organised or managed by Tzu Chi and ought not be treated or deemed to have been organised or managed by Tzu Chi under any circumstances whatsoever.

The personal information and/or data that being referred to in this consent form shall refer to both sensitive and non-sensitive personal data in accordance with the Personal Data Protection Act 2010 (PDPA).

By clicking "Submit" in this form, I agree to participate this program voluntarily knowing the above and further agree to the following terms and conditions:

1.Personal Data Protection: All Personal Information will be kept confidential but may be disseminated within relevant Tzu Chi Volunteers - Organizing Committee ("the Organiser") in organising this program. The information will be strictly treated and handled in compliance with the PDPA. Any photos, recordings (audio or video) and written materials created for and/or during Healthier Me Challenge 21 are proprietaries of the Organiser and may be used for promotional purposes at the discretion of the Organiser. I hereby understand, agree and give my consent to the Organiser to collect my Personal Information for disseminating more information on the Healthier Me Challenge 21, and, from time to time, be called, sent SMS-es, text messages, or emails on more details regarding Healthier Me Challenge 21 and other events organised by the Organiser, as well as the relevant means and terms of registration and participation.

2.Release of Information: Notwithstanding the program is not organised or managed by Tzu Chi, I hereby understand, agree and give consent that my Personal Information may at the discretion of the Organiser, to be released to Tzu Chi and its volunteers for internal use, which may include but not limited to:-

a.medical related details, i.e. Blood test, Blood Pressure, Weight;
b.relevant personal details which Tzu Chi may require;

The Personal Information may be made accessible by relevant or designated Tzu Chi volunteers and staffs or personnel assigned. The Personal Information may be retained and subject to my prior consent, partly be shared with the public via Tzu Chi’s operated media or organized activities as motivational stories, research or training.

3.The impact of the program differs and varies in accordance with circumstances from time to time. Hence, I fully understand the effect and consequences in participating this program and hold myself full responsibility on the effect of the program on my personal health. Knowingly that I agree to participate in this program voluntarily and understand all the above terms and disclaimer, I hereby indemnify the Organiser and Tzu Chi for any claim, demand or losses arising, occurred or incurred from my participation in this program.
