Covid 19 Health Self Assessment Form - April 27 SAT exam
PLEASE NOTE:

1) SRVUSD must receive this acknowledgement form by the morning of April 27 in order for the student to take the SAT exam.

2) If a student is not feeling well while on the high school campus the student will be re- administered the self assessment and if necessary sent home.

3) Students must wear their mask at all times, including during the test, while on campus.

4) Any student who violates the mask process will be sent home and disqualified from completing the SAT test.
