Umpire – Hold Harmless Agreement

The umpire listed below is named candidate for a position as an umpire in the Dumfries District Little League program (aka Dumfries, Triangle, Quantico Little League (DTQLL)), hereby give my approval to participate in any and all umpiring activities.  

Note: If a parent (adult) and child are both umpiring there will need to be forms for each on file.

1) I know that umpiring baseball may result in serious injuries and protective equipment does not prevent all injuries to umpires, and do hereby waive, release, absolve, indemnify, and agree to hold harmless the Dumfries District Little League, the organizers, sponsors, supervisors, and participants from any claim arising out of any injury to myself or child whether the result of negligence or for any other cause.   *

2) I agree to return upon request the umpire uniform and other equipment issued to myself or my/our child in as good a condition as when received except for normal wear and tear.   *

3) I agree that I and/or my child (if listed as a junior umpire) will be required to attend an umpire training session sponsored by the Dumfries District Little League and be familiar with the rules of the game.   *

4) I agree that I and/or my child (if listed as a junior umpire must be present or secure a registered replacement when scheduled to umpire. Failure to meet these requirements may result in forfeiture of the child's opportunity to umpire in Dumfries District Little League games. *

5) I agree that I and/or my child (if listed as a junior umpire) is expected to demonstrate good sportsmanship and conduct themselves in a professional manner when umpiring.   *

6) I agree that I and/or my child (if listed as a junior umpire will serve at the will of the Dumfries District Little League Board and its Umpire Coordinator and can be removed at any time for due cause. Signing this waiver form does not guarantee or promise game assignment(s) with Dumfries District Little League. By signing below, I confirm that I have read, understood and agree to the provisions of this form; the potential dangers involved in umpiring; and waive any claim which may arise, as provided herein.   *

**If you are 18 years of age or older**                              As a condition of volunteering, I give permission for the Little League organization to conduct a background check on me, which may include a review of sex offender registries, child abuse and criminal history records. I understand that, if appointed, my position is conditional upon the league receiving no inappropriate information on my background. I hereby release and agree to hold harmless from liability the local Little League, Little League Baseball, Incorporated, the officers, employees and volunteers thereof, or any other person or organization that may provide such information. I also understand that, regardless of previous appointments, Little League is not obligated to appoint me to a volunteer position. If appointed, I understand that, prior to the expiration of my term, I am subject to suspension by the President and removal by the Board of Directors for violation of Little League policies or principles.                          ***IMPORTANT -- JDP background investigations; the email may have header that says "Dumfries District Little League". *
