Please fill out this short questionnaire to get started with 4th-5th Grade TO BE READ. This service is for kids in grades 4-5 only.

Sign up by the 7th of each month to receive your bag. Pickup is the 15th - 22nd of each month. *You must fill out this form to sign up for a bag each month in order to receive a bag each month.*

Each bag will include at least 2 library books personally picked out just for you (due the 15th of the next month) + themed snacks/goodies.

The more you tell me about your preferences below, the better I can match you with books you might like!

You will get an email when your bag is ready for pickup the 15th of the month. You will get one week to pick up your bag. Please let library staff know if you are unable to pick up your bag within that time, so that we can hold it back for you.
