All participants in these events must sign and turn in this waiver of liability prior to competing. Athletes under age 18 must have the signature of a parent or legal guardian. Coaches entering multiple athletes will sign one waiver for all athletes entered.

I agree to waive and release any and all claims I may have against all sponsors, officials, volunteers, assistants and employees of this meet including Independence High School, East Side Union High School District and San Jose Track Club for any damages or injuries I may suffer as a result of my participation in this meet.

I understand that there will be no medical services or training staff available at this meet. I affirm that my physical condition and fitness are adequate for me to participate safely in this meet.

I agree to release my name, photo, and/or video for publicity purposes. I understand that my name, division, likeness, and results may be published online.
