Please fill out this form and submit.  Make sure you include your payment type and send that in to be fully registered. Registration fee is $70, all books and a t-shirt is included, stands are $15.  After September 2, 2024 the registration fee will increase to $80.  Classes start the week of September 9th.

PARTICIPATION AGREEMENT
I, the undersigned, am a parent or legal guardian of the named minor above. I understand that my child will be participating in the Mountain West String Academy Elementary Orchestra and that my child is expected to arrive on time to class each day prepared with orchestra materials and I will support practicing at home. If my child is attending an after school class I will pick up my student on time. I also understand that there are rehearsals and performances during the year that my student must attend.
I will allow MWSA to distribute contact information for MWSA program purposes only, this includes fund-raising efforts within the program. I will allow photos and video of my child in the classroom and concert setting for calendar fund-raising purposes, classroom, and concert activities.

Registration deadline is September 2, 2024 by noon.  After that time, late registration will be $80.
