Phone Kept at Home Agreement
I attest that my student will not bring a phone or other electronic device into Stevens High School.
I understand that if my student is found to be using, or having a phone out during school hours, the student will  receive one full day of out of school suspension for the first offense, two days of OSS for the second, and five days of OSS for the third offense.  Any student forgery of this document will result in immediate consequences.  
All forms will be verified through a phone call from the school before putting in place.
