Dear Parent/Guardian:
As part of the HUSD #3 Instructional Time Model (ITM) that allows students to conduct virtual learning during district designated ITM days due to circumstances such as inclement weather, sickness (with doctor's note) or other emergency measures declared by HUSD #3 that may prevent students from attending in-person attendance at school, each parent is required to verify academic attendance for at-home learning.
To accomplish this verification, we ask each parent to complete this Google Survey to verify the attendance of each child while on an ITM schedule. Attendance verification will be required for each ITM day.
Please complete the form and submit so your child's attendance can be verified as "present". In addition to this verification form, your child will be required to submit the required school work/assignments for the instructional day to be counted as present.
