Johnson Elementary - Pre-arranged Absence Form
The Huron Valley Code of Conduct states, "Families should plan vacations during the periods when school is not in session."  Teacher instruction time cannot possibly be replaced by make up work.   New skills and learning processes will be missed.  

If you feel that this vacation is necessary and outweighs classroom instruction time, you may request a pre-arranged absence.

  1.   This form will be printed and a copy will be included in the student's record
  2.   Make up work should be requested.  It will be assigned at the discretion of the classroom teacher.       Please give your teacher ample notice to get your work together.
  3.   Any assigned work should be submitted upon return to class.
