Bayou Academy fees cover ONE BOOK per family - you have already paid for this in your enrollment fees. This form is for EXTRA BOOKS ONLY, if you prefer more than one book for your family.

Your included book will be given to the OLDEST sibling.

You MUST fill out the form for EACH extra book needed. This is for the 2023-2024 book that will be passed out NEXT school year. (August 2024).

Example: If you need 3 extra books, you fill this out 3 times.

***Do not include a form for your OLDEST student.***

This is the ONLY time yearbooks will be offered. We will most likely not have many, if any, extras.
