Our Mission is to improve wellness and resiliency to equitably impact the lifelong health of all Frederick County residents. Our current community health priorities are addressed in 3 Local Health Improvement Workgroups: Adverse Childhood Experiences (ACEs)/Infant Health, Behavioral Health, and Chronic Diseases. Anyone interested may join a Workgroup!

Every 3 years, these priority areas are reevaluated through the Community Health Needs Assessment (CHNA)/Local Health Improvement Process (LHIP). The Coalition works with the Frederick County Health Department, Frederick Health, and community partners to reassess community health needs. This process includes a community survey, targeted focus groups, and secondary data sources. Partners and community members provide additional feedback on the Top 5 priorities during a Public Input Event (Wed., Jan. 19, 2022, 2-4 pm), and then the Board selects the top health improvement areas. Workgroups are formed for each of these priorities and, for the next 3 years, the Workgroups work collaboratively to identify actionable areas of improvement and develop interventions to to address those needs.  

Improving our community's health is best achieved when all sectors, groups, and individuals in the community partner and work together. By completing this survey, you will help us learn more about your areas of interest and how you may want to contribute to our mission. There are opportunities to get involved in the CHNA planning process that is happening right now and/or to participate in one of the current Workgroups or the new Workgroups active in 2022. Or, maybe you just want to stay informed.
