Young Adult Literature Permission Form (All Grades)
"Young Adult" (YA) literature is typically categorized as literature that is recommended for ages 12 and up.  While YA literature is typically written for ages ranging from 12 to 18 years, marketing attracts readers between the ages of 10 and 25.  YA literature's plot, theme and setting are appealing to students.  The subject matter addresses problems, issues, and life circumstances of interest to middle and high school readers.  That said, the content is typically intended for a mature reader.  

By completing this form, you are giving your child your approval to check out "any" of the YA titles or only "specific YA titles" in the MBMS Media Center that you approve.  (If you select "specific titles", your student will be given a form for you to sign with the title and author of the YA book your student wants to check out.  Once it is returned with your signature, the student can then check the book out).  All of the YA books in the MBMS Media Center are clearly labeled with YA stickers.
