Thank you for helping by filling out a recommendation for a potential math or stat major Undergraduate Teaching Assistant (UTA).  Your answers to these questions will help us to evaluate the suitability of the applicant.  

Please fill out a separate form for each applicant who requests a reference from you.

Duties of the successful applicants will include (but are not limited to):
    * Participating actively in the weekly UTA seminar (tentatively scheduled for Tuesdays at 5:30pm - 6:30pm).
    * Working at most 5 hours per week with the course and the course instructor.
                Responsibilities may include creating study sheets, leading review sessions,
                helping in the classroom, grading, holding office hours (these activities are planned
                with the mentor). Participants are expected to meet every week (or every other
                week) with their mentor to discuss performance and progress.
    * Working 2 hours per week in the Mathematics Tutoring Center (Calculus tutoring)
    * Writing a report at the conclusion of this experience suitable for publication on the
       Department of Mathematics web-page.
