How to contact the claimer *
Most frequently fests will ask writers for their emails, in order to contact them regarding confirmation, check-ins and submission. You could ask them for other ways to contact them (social media handles, for example) if you feel that might work better.
Usernames on Posting Platform *
Consider whether you will ask your writers to have an AO3 account prior to claiming. If you do, it might be useful to ask writers for their AO3 handles here. If you accept fanart claims, consider how you will proceed in that regard, and whether you will ask fanartists to provide their handle/platform here.
Anonymity *
Some fests offer to keep works anonymous even after reveals. If you decide to ask that, do ask. In the case of fanartists, it might be the case you would have to post their work in their stead.
Kind of Content *
Consider whether you will accept both writer and fanartist claims. If you do accept both, you should ask it somewhere in this form.
First Prompt *
Usually, fests ask claimers to list their three preferred prompts, in case the claimer's first choice ends up being chosen by someone else first. Also consider whether you will accept content creators to claim their own prompts (in case they participated in prompting).
Second Prompt *
Some fests set the second and third prompt choices as obligatory, whereas other fests allow claimers to just mention one prompt. Consider whether this step and the next will be obligatory in your fest.
Third Prompt
Same as above.
Self-Prompt
Most fests that allow self-prompting ask for content creators to offer a prompt for their own work. Consider whether you will accept self-prompting, and if you do, whether it will be subject to the same deadlines as claiming.
Submit
Clear form
Never submit passwords through Google Forms.
