PARENT RELEASES
By signing electronically below, I certify that my child/children is/are allowed to attend the AWANA Program at Real Hope Lacey.  Club starts at 6:30 pm and drop off begins at 6:10 pm each day. Club ends at 8:00 pm each week.   I understand it is my responsibility to pick up my child by 8:15 pm each day during the club week.

As with any physical activity, there is a risk of injury. I fully accept this risk and hold harmless from any legal liability, Real Hope Community Church and any persons involved in the Awana club ministry.

By signing electronically below, I agree that in the event of an emergency that requires medical treatment for the above-named child/children, every effort will be made to contact me or my emergency contact. However, if I/we cannot be reached, I give my permission to the Real Hope Staff / Awana volunteers to give emergency treatment that could include first aid, CPR by a qualified person at RHCC and/or secure the service of a licensed physician to provide the care necessary for my child’s well-being. I also give permission for my child to be transported by ambulance or aid car to an emergency center for treatment. I assume responsibility for all costs connected to any accident or treatment of my child and waive my right of informed consent to such treatment if unable to be reached.

Costs and Fees:
*There is a registration fee of $30 per child with a max of $70 per family.  Registration fee may be divided into 3 monthly payments as needed for a family budget.  

*Each clubber is required to have a book for their level which costs $18.  This will be due when the clubber starts the book (all clubbers start with a free introductory booklet)

*Club T shirts/Vests and Awana Backpacks are optional and available for purchase.  T Shirts are $20-$30 depending on the clubber's level.  Backpacks are $10-$20 depending on the style desired.  All T shirts and Backpacks are ordered as requested.

If at any time, a club family faces financial difficulty and struggles with the registration or book fee please let us know as there are programs/scholarships in place to help offset these costs.  We do not want costs to hinder a family from attending our AWANA program.
PHOTO RELEASE AND INTREST IN HELPING
OPTIONAL PHOTO RELEASE:
Real Hope Lacey may from time to time document the activities of the ministry with photos and /or video.  These are used to both remember the event and help promote future events through Real Hope Community Church.  By agreeing below, you allow us to include your child in these photos/photos.  If you do not agree, your child will not be included or will be obscured in these photos.
