This form should be completed separately for each school or program where the transcripts are to be sent. (i.e. 3 applications= form should be completed 3 times)

There are currently NO transcript fees for current students. The Seventh Semester grades will be sent automatically in Mid-February when available. The Final Transcript must be requested through the Senior Survey. You can check the status of transcripts sent on Naviance.

SCHS cannot guarantee on-time delivery for requests submitted late.

1) I have turned in the “Consent for Release of Student Records in Support of Post-secondary Applications” form, and e-mailed it as an attachment to the transcript office at SCHStranscripts@fcps.edu. (This waiver will only need to be completed ONCE, with your first transcript request.)

2) I have completed and submitted the Request for Counselor Recommendation Form and the Parent Input for Counselor Recommendation Form. These forms should be e-mailed as an attachment to your school counselor.

3) The transcript package includes a current transcript, a Secondary School Report with Recommendation completed by the counselor, and a School Profile. The Transcript Office and your counselor cannot guarantee delivery of these items by the deadline for requests that are submitted late and do not adhere to the deadlines outlined above.

Please Note: South County HS does not send official standardized test scores (SAT, ACT) to colleges. You must request them online via College Board (www.collegeboard.org) or ACT (www.actstudent.org).
