Student must fill out this form completely to request a Verification of Enrollment.
First Name and Last Name of the student must match student's Birth Certificate and Skyward account. Please include full student ID#.
Front Office will check eligibility of student requesting VOE with Attendance Officer.
Processed VOE's will be ready to pick up in the Front Office / Receptionist desk.
