Tuition Prices & Discounts

Each class will be $12 per class, OR bundle 2 or 3 classes to receive 5% off of the first additional class and 10% off the second additional class.

For an example:

Class 1 - Ballet & Jazz Combo = $12/class
Class 2 - Acro = $11.40/class
Class 3 - Hip-Hop & Tap Combo = $10.80/class

All pricing is plus HST.

Liability and Assumption of Risk (MUST CHECK ALL): *

I understand and agree that participating in any dance lesson, class, workshop, rehearsal or performance, there is a possibility of physical injury or illness, including COVID-19. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury, accident or illness, which might occur to my child, or anyone in his/her family, during any of Titanium Arts Lab classes, lessons, rehearsals, performances, or activities.

I understand that in relation to COVID-19, Titanium Arts Lab is following the latest guidelines issued by the Government and Health Unit and will continue to offer in-studio classes with simultaneous off-site instruction through Zoom. I understand if my child, or anyone in his/her family is sick, they will not come to class, and must have a doctor’s note or negative test result clearing them from COVID-19 before they can participate or stay out of dance for at least 14 days symptom-free, whichever comes first. I understand I am willingly and voluntarily allowing my child to participate in an activity where they are around other people, thus increasing their risk of contraction. I understand the risk associated with allowing my child to participate in dance classes regarding COVID-19.

I understand Titanium Arts Lab will do whatever it can to prevent an accident, injury or illness, therefore, I exempt, release and indemnify Titanium Arts Lab, its owners, directors, instructors and/or other students from any and all liability claims whatsoever, arising from any damage, loss, injury, or illness of my child or other family member(s) which may arise as a result of the student participating in events sponsored by Titanium Arts Lab.

WEBSITE AND SOCIAL MEDIARELEASE FORM:

From time to time, Titanium Arts Lab and its directors and instructors will post photographs, videos and other items to the company’s website, as well as different social media platforms, for the purpose of marketing and celebrating your child’s achievements.

I grant permission to Titanium Arts Lab to post my and/or my child’s story, photo, or other items, herein referred to as Materials to its website www.titaniumarts.com and social media platforms such as Facebook, Twitter, Instagram, Tiktok, etc.

I hereby release Titanium Arts Lab and its owners, directors, and instructors from all claims and demands arising out of or in connection with any use of said Materials, including without limitation, all claims for invasion of privacy, infringement of my right of publicity, defamation and any other personal and/or property rights.

I acknowledge and agree that no sums, whatsoever, will be due to me as a result of using the Materials or any rights therein.

I acknowledge that my child is under 18 years old and lacks the legal capacity to enter into binding agreements. Accordingly, I have read this release and consent to my child’s inclusion in the Materials and will not contest the rights granted in this release.
