September - February CECMP Alternative Pathways CoP - Request for Registration 
Thank you for completing the following registration to request to attend our six-session Community of Practice (CoP.) Each registration will include one night's hotel room for the evening prior OR the night of the to the in-person CoP, for those traveling more than 50 miles to Chabot College, 25555 Hesperian Blvd., Hayward, CA 94545. The September 23 and January 6 session will be held in the Event Center, Building 700.

Reimbursable expenses will include: 
Travel
1.  Your roundtrip travel to and from the CoP. This includes travel by flight, train or car – whichever is the least expensive mode of transportation (take a screenshot to document least expensive mode) – to and from the CoP, beginning at your work or home – whichever is closest. CECMP will only reimburse for Lyfts, Uber, with a single stop, if this is the only and least expensive means of transportation (to/from an airport).  This is a reimbursable travel expense at a rate of $.655/mile. A printout displaying miles traveled is required to receive reimbursement.     
Meals
2.  A continental breakfast and lunch will be provided September 23rd and January 6th. One dinner expense is covered for those staying at the hotel.   Participants must provide an itemized receipt in order to be reimbursed. No alcohol may be purchased for reimbursement and tip is not included for reimbursement. The state maximum reimbursement rate for dinner is $23. We are unable to reimburse beyond that rate. 

Please note, we are only able to accommodate special request for vegetarian based meals. If your dietary needs are more complex, please arrange to bring your own meal. Thank you for your understanding.
