Community Event Outreach Request
If you are planning on attending a community event with your animal as a representative of the UMass 4-H Extension Program, you must obtain prior approval from your local 4-H Educator at  least 24 Hours prior to the event.

All animals must have the proper health certification as required by Massachusetts Division of Animal Health. Please complete the form below at least 24 hours prior to the event date; if you require an insurance certificate 2 weeks notice will be needed. Thank you- Plymouth County 4-H office.

All participants need to be enrolled members or volunteers.

Please click the box to indicate that you both understand and agree to the following UMass Extension 4-H policy regarding having club members' animals in public when representing 4-H:     1) 4-H is an educational program and this includes engaging in opportunities to educate the public on animals.   2)  It is very important to understand, however, that the public should not be directly feeding animals, nor should they be left unattended with any animal during the event.      3)  If your educational program requires the public to interact directly with any of the club members' animals in order to meet its educational objectives, this plan needs to be approved in advance by your local county or regional 4-H extension educator.    4) I both understand and agree to the UMass Extension 4-H policy regarding the interaction of the public with animals belonging to 4-H members when our club or individual club members are representing UMass Extension 4-H at any public events. *
