The Harlan Elementary School Counseling Program would like to survey parents, teachers, and students in order to develop and provide programs that meet the students' needs. This survey will help us determine the needs of the students individually as well as which topics we need to address in the classroom and small group sessions. The survey is anonymous, as we require no identifying information other than grade level unless you choose to add your student's name. We would appreciate your time answering the 5 questions below. The school counseling department would like to facilitate small groups throughout the school year as needed. If you would like your student to be part of a small group, please include his or her NAME and the TOPIC OF INTEREST below. You can choose from the lists provided above or choose your own topic. Examples might be study skills, healthy friendships/relationships, anger management, academic motivation, dealing with divorce, grief, etc.

As a parent or guardian, I would like to attend a parenting workshop on the following topic. Examples might be online safety for kids, Math Night, how to help your child during divorce, etc.:
