This form opens at 1900hrs (7pm) on Monday 11 April 2022. Any submissions made prior to this time may be disregarded.

The closing date for submissions is 1700hrs (5pm) on Thursday 21 April 2022. Teams will be selected on a first-come, first-serve basis, with priority given to LSL teams. Should there be spaces at the closing date and time, non-LSL teams will then be selected to fill those spots via order of application.

We aim to have four divisions, but the final structure will be determined based on registration numbers and pitch availability. Teams will be assigned to divisions by the LSL Committee based upon ability. If the team you are entering is likely to be significantly different in ability to that in the League (e.g. inclusion of additional beginners or a mashup team), please indicate this in the notes.

Tournament entry costs £170 per team. Payment is required upon completion of this form via direct bank transfer (details below). Your space on the tournament is not secured until payment is received.

We will be holding the after party at the Greene Man pub, so do encourage all your team to join us after the tournament! Further details will be published in due course.
