The Puesta del Sol PTA would like to celebrate our students’ and staffs’ accomplishments and perseverance with a “Special Edition” Puesta del Sol t-shirt. This shirt is free to each student and staff member.

Please complete this form to let us know your student's name, grade, teacher, and shirt size. If you have multiple students at Puesta del Sol, submit a separate form for each student.

DEADLINE: Please submit your student's form by 5:00pm on Tuesday, May 10th.

(TEACHERS & STAFF - Please enter your name in the Student's Name field, the grade you teach (or put Staff) in the Grade field, and your Last Name in the Teacher's Name field.)
