Players with a birth date within one year of the age group cut-off may request an age group adjustment to play UP in an older age division.
- Adjustment is not guaranteed.
- Adjustments must be requested EACH SEASON, we do not extend from one season to the next.
- Adjustments are dependent on available space.  

All players requesting adjustment must follow this procedure:

1. Register and pay in their true age group.
2. Complete this form.
3. Be sure to use an email you will check frequently. You will receive a confirmation of this form submittal.
4. Upon receipt of form, CSA will review it and send an age group adjustment waiver for parent signature.
5. Player movement is not considered until waiver is received by email to CSA.
6. You are responsible for any additional fees and they must be paid prior to team assignment.
