If the diagnostic tool above points you to register at the HDSB Welcome Centre, then please complete this form by submitting all the required supporting documents to begin the registration process at the Halton District School Board (PUBLIC). If you wish to register your student at any other Board (e.g. French, Catholic, private), please contact them directly.
Please do not fill out this form until you have arrived in Halton, Ontario, with a finalized address.
