Application for Stay-and-Play Forest Pre-K/ Kindergarten
Please fill out additional form for each child between 3-6 applying for this program. The form is an application form for Stay-and-Pay Forest Pre-K/Kindergarten taking place weekly every Thursday from 1pm-3pm for caregivers to attend with their 3-6-year old children. Tuition is $150 per month. Younger siblings 2 and under attend free. If you have questions and are not ready to apply yet, please fill out our inquiry form instead, or email Rachel@freehavenfs.com. To complete your application, please pay the $25 application fee.
