Release of Liability

I/we certify by accepting these terms and conditions that the applicant named has my/our permission to participate in the Black Star sports programs (including any Speed & Agility camps, skills camps, skills evaluation, and tryout sessions) and I/we assume all the risks and hazards associated with such participation, including transportation to and from Black Star activities. I/we hereby waive, release, absolve, and indemnify Black Star's officers, directors, commissioners, managers, coaches, supervisors, participants, and persons transporting my/our child to and from Black Star activities, from and against any liability for, or claim arising out of any accident or injury to said applicant occurring during the course of his/her participation in such activities or his/her transportation to or from such activities including by not limited to the sole or concurrent negligence of the Black Star, its officers, directors, commissioners, managers, coaches, supervisors, umpires, referees, participants, and persons, participants and persons transporting my/our child to and from Black Star activities. 

I/we hereby also waive, release, absolve, and indemnify any trainers, instructors, coaches, and organizations engaged by Black Star (regardless of whether paid by Black Star) to assist with or operate any Black Star sports program, Speed & Agility camp, skills camp, skills evaluation, or tryout session, from and against any liability for, or claim arising out of any accident or injury to said applicant occurring during the course of his/her participation in such activities. 

I/we certify that it is understood that my payment is an enrollment fee only and the registrant will be assigned to a team determined by the Black Star at its sole discretion. 

I/we authorize the coaches of my child's team to act for me according to their best judgment in any emergency requiring medical attention. 

Code of Conduct

We (Player & Parents/Guardians) further agree to abide by ALL Black Star rules and to conduct ourselves in a sportsmanlike manner during all Black Star related activities. We further agree to abide by all directions and decisions of the referees, coaches, and officials of the Black Star Program. We realize that violations of the above are consequential and could result in suspension(s) for one (1) or more games, practices and events.

Media Waiver 

I/we give my/our consent for photographs taken of me or my child during Black Star games or events to be used for publicity purposes: on the Black Star website, in brochures and flyers and news releases, and in presentations to future prospective program participants. I/we understand that I will receive no compensation for such uses. I/we retain the right to have any photographs discontinued from use in any or all of the above venues upon request, and if, at any time, I/we wish my/our photograph(s) to be discontinued from any of the above, it is my/our responsibility to contact the program director to make this request.
