2022 MVC Girls Events Player Registration Form (For Players 15-18 yrs old)
Please complete this form and let us know what events your player will be participating in. DATES for events are listed in the form below.

This helps MVC appropriately staff our events and notify registered players of any changes. This form is only to be used to register for our upcoming girls open gyms and open house/eval nights (Players competing in club in the 15s, 16s, 17s and 18s age groups). We have a separate form for players who are 14 and younger.

*Open Gyms are great opportunities for players to come get court time/drills and gameplay with our coaches and other players. Open gyms are 2 hours and $20 cash paid at the door.

PLAYERS CURRENTLY PLAYING ON HIGH SCHOOL VOLLEYBALL TEAMS ARE NOT ELIGIBLE TO PARTICIPATE UNTIL THE TEAM IS ELIMINATED FROM PLAYOFF CONTENTION. Please still register your player so we can track number of players interested and ages.

*Eval nights are opportunities for players to meet coaches and be evaluated prior to the upcoming club tryouts. Teams are not formed during evaluation nights and contracts will not be offered to any player prior to tryouts. Open Houses/Eval Nights will be FREE this year!

This is NOT the form you will use to register for club season tryouts. Tryout registration will be open at a later date. All club season info will be posted sometime in October.

**ANY PLAYER WHO IS BORN ON OR AFTER JULY 1, 2008 FALL INTO THE 14 & UNDER AGE DIVISION OF USAV AND ARE ELIGIBLE TO REGISTER FOR THESE OPEN GYMS AND TRYOUTS.

**ANY PLAYER BORN ON OR BEFORE JUNE 1ST 2008 WILL BE REQUIRED TO PLAY 15s DURING THE CLUB SEASON AND SHOULD BE REGISTERING FOR THE HIGH SCHOOL GIRLS OPEN GYMS AND TRYOUTS. THOSE PLAYERS ARE NOT ELIGIBLE TO PARTICIPATE WITH USAV IN THE 14 & UNDER AGE DIVISION.
