ข้อตกลง และเงื่อนไข *
1. ข้าพเจ้าขอรับรองว่าข้อมูลข้างต้นเป็นความจริงทุกประการ และขอยืนยันว่าข้าพเจ้า มีสภาพร่างกายสมบูรณ์แข็งแรง มีความพร้อม และสามารถลงแข่งขันในประเภทที่ลงสมัครได้ 2. ข้าพเจ้าจะปฏิบัติตามกฎ กติกาทุกข้อ โดยไม่เรียกร้องค่าเสียหายใดๆ หากเกิดอันตราย หรือบาดเจ็บแก่ตัวข้าพเจ้า ทั้งก่อน และระหว่างแข่งขันข้าพเจ้ายินดีแสดงหลักฐานเพื่อพิสูจน์ตนเองต่อคณะผู้จัดการแข่งขัน 3.ข้าพเจ้ายินยอมให้ผู้จัดถ่ายภาพหรือภาพยนตร์บันทึกการแข่งขัน และถือเป็นลิขสิทธิ์ของคณะผู้จัดการแข่งขันครั้งนี้ 4.ข้าพเจ้ายินยอมที่จะไม่เรียกร้องใดๆหากมีกรณีเปลี่ยนแปลงหรือแก้ไขวันเวลา กำหนดการ รูปแบบทุกอย่างที่เกี่ยวกับงาน 5.ในกรณีที่กิจกรรมนี้ต้องยกเลิกทั้งหมดหรือส่วนใดส่วนหนึ่งโดยสืบเนืองจาก เหตุสุดวิสัยใดๆทางธรรมชาติหรือภาวะอื่นใดก็ตาม ข้าพเจ้ารับทราบและยินยอม ว่า จะไม่มีการคืนค่าสมัครให้แก่ข้าพเจ้าทั้งสิ้น 1. This waiver and release statement certifies that I am medically fit to compete in the event and fully understand that 2. I enter at my own risk. The organizers will not be held responsible for any injury, illness of loss, during or as a result of the event. Further, 3. I consent to the Organizing Committee recording this competitive event through photography and film, and that the Committee is the sole owner of the copyright to those pictures and films. 4. I agree to not claim if there is a case to change or modify the date, time, schedule, form, everything related to the event. 5. I hereby agree that in the event of the event being cancelled in whole or in part due to win, storm, rain, inclement seas or weather or other condition, my entry fee shall not be refundable.
