I am requesting your cooperation in completing this family survey that will help us secure the most federal and state funds (which supports educational programs, technology and textbooks, building improvements, etc.) for the children in our Catholic schools. I want to assure you that your privacy will be protected and that no names are required on this survey. Each form is confidential. Only the address and grade level are shared with the school district so that it can determine funding for our school. Please only submit one form per family, but be sure to clearly list grade levels for all of your children attending our school. Please answer all questions below.
