Please complete this form if your student is currently a remote learner and intends to return to the physical building for the third quarter. You will need to submit a form for each student who will be returning. The second quarter begins January 5, 2021.

This form is NOT for:
1. Students who are not going to return at the beginning of the second quarter.
2. Traditional students who are currently coming to school every day.

The deadline to submit this request is Friday, December 11, 2020.

*Please Note - Students who will be transitioning from remote to traditional learning will stop by the front office on January 5, 2021 to receive their schedules!
