I, the undersigned, do hereby authorize Rabbi Gerald C. Skolnik of the Forest Hills Jewish Center to be the agent for the sale of all Hametz in my possession, so as to fulfill the Torah’s command to have no Hametz in my possession during the festival of Passover. I am including all the relevant names addresses, home and office, relating to me personally, as well as the names and addresses of anyone in my family who is depending on me to do this for them.
