Covid-19 Status update -  WUCC
This form is to officially inform WFDF and the WUCC LOC about any possible covid cases while you are at the event. Information on this form will assist us in tracking your health updates and to contract trace any close contacts during the event.

You should fill out this form if you at any time during your trip to the United States of America, or throughout the event itself:
a) are displaying symptoms of active Covid-19
b) test positive for covid (RAT or PCR test)
c) are a close contact of someone that tests positive to Covid.
d) if your status has since changed
Anyone showing symptoms of the virus or with a positive test, must immediately self isolate and inform WFDF via this form. Do not come to the venue, we will be in touch with you to discuss the next steps.

Only recovered participants with an official negative RAT test (return of play must first be authorised by the WUCC on-site Chief Medical Officer ) will be allowed to attend the event after recovering from Covid.
