This Application must be completed ANNUALLY to be considered for ongoing support & services. Completion of this application does not guarantee ongoing support. All extensions are contingent on eligibility, engagement, reporting compliance, and the availability of funding.
SERVICE EXTENSION DETAILS
- GroundWork core services are 100% paid for by our sponsors, and FREE to the entrepreneur.
- Specialty services & referrals may require an engagement fee, but utilizing these services is entirely optional, and we will discuss any fees in advance.
- To be eligible for an extension, you must complete the Service Extension Application (this form) as well as the Client Annual Survey, another short survey that is due each spring to share your business results for the previous tax year.
- You must complete your most recent Client Annual Survey before we can review your application. If this is an issue for you, we will notify you.
- Depending on your enrollment date, the collection period for these two surveys may overlap.
If you choose not to apply, are not eligible, or we do not have available sponsorship for you, you can still receive up to 5 hours of FREE support from Livelihood NW annually through the
Oregon Women's Business Center or our
Open Business Advising program.
