St. John's Lutheran Church of Lakeville has a vibrant Care Ministry, which provides spiritual and emotional support for individuals who are physically unable to attend worship due to health concerns, aging, or mobility limitations. In order to best care for these individuals, St. John's would like to connect with a Family Member Contact, someone with whom we can be in conversation about your loved one's current needs, and stay informed of any changes to their health or location.

If you are a family member of someone who is currently being visited through St. John's Care Ministry, or have a family who may be interested in Care Ministry visits, please complete this form. All information provided will be kept confidential.
