Please complete this form if you are interested in having your child attend CYC Summer Camp. Children who are currently enrolled will have FIRST PRIORITY. If we have more interest than spots available we will have a lottery on January 22nd. Completing this form does NOT register your child for camp, but simply enters your child's name into the lottery, should we need it. If you're child does not currently attend CYC we will place you on the waitlist after we have done the lottery for currently enrolled families.
