CONSENT TO RECORD AND DISTRIBUTE

I agree with the following statements:

I hereby grant CoreAalst permission to photograph, record, videotape and/or audiotape me for use in CoreAalst's educational and training seminars.

I understand that CoreAalst may now or in the future retouch or edit the photographs, recordings, videotape, or audiotape (hereinafter referred to collectively as the “Recordings”) as necessary in its sole judgement for proper presentation.

I waive the right to approve or inspect any of the Recordings made pursuant to this Agreement.

I understand that CoreAalst may distribute the Recordings widely, including in the US and other countries, for the uses described above.

I also understand and agree that such Recordings may be retained by CoreAalst for an indefinite period of time in its libraries, files and/or on its web sites, and thus may be subject to continuing use and disclosure for the purposes described in this Consent.

I will not hold CoreAalst, or its agents, employees, affiliates, successors and assigns liable for their use or distribution of the Recordings and I do hereby release CoreAalst, and its agents, employees, affiliates, successors and assigns from any and all claims for damages for libel, slander, invasion of privacy, or any other claim based on the use or distribution of the Recordings. In granting this consent, I understand and agree that no monetary or other consideration is owed to me by CoreAalst or any other party.
