Thank you for your interest in the Senior Farmers' Market Nutrition Program.
To qualify for the program, you must:
1. Live in Butler, Clermont, Clinton, Hamilton or Warren counties in Ohio.
2. Be age 60 or older.
3. Have an annual household income that meets one of the following guidelines:
-1 person in household, annual income of $0-$23,828
-2 people in household, annual income of $0-$32,227
-3 people in household, annual income of $0-$40,626
-4 people in household, annual income of $0-$49,025
-5 people in household, annual income of $0-$57,424
-6 people in household, annual income of $0-$65,823
Please continue and fill out the application if you believe you meet the above program qualifications. Note that each eligible applicant in a household must complete a separate application.
