Please complete the form below to select a time and date to collect your Yr11 and Y13 exam certificates. They will be distributed at the security hut.

Please bring a photo ID with you when you come to collect as without ID you will not be given your certificates.

Please at least 24hrs for the exams department to process your request. On the day request will not be processed so please email the exams emails below if it will be processed on the day as you do not have a wasted journey.

If you have selected a slot and are not able to attend, please inform the school on: exams@mhchs.org.uk
