A few things to note:

-If you ask for us to check back in with you, please understand that it may take a day or two. Be sure to utilize/see other resources at the end of this form if you are feeling you need more immediate mental health help.

- If you provide a phone number as your requested communication tool, please understand that you will be getting a call from us on a local number- however, it is is a Google Voice number... and not our actual home phones. We intend to treat it as an office number where you can return a missed call, leave a message and we will address your needs/concerns within 48 hours. It is not an emergency line.

- You can always email your counselor directly (kpmiller@pcsd1.org or tglee@pcsd1.org) if you feel there is additional information you'd like to send/share with us.

-We are both taking care of our kiddos at home, so we may have to work around life to help meet your needs, please be patient with us :)
