Below is a digital version of our Library Permission Form. Please fill out info below so we can have clear communication about missing Library books. Every student who has this form filled out online will be entered to win a Prize Backpack of books from Mrs. Keeling. Thank you for taking the time to fill out this form. Please fill out this form for EACH of your children. Names will be sorted by teacher so 1 Name per form please. Sorry!
