Twirl Field Trip Agreement
In submitting the Field Trip Request form, you acknowledge that you have read and agreed to the rules and terms outlined below. Additionally, you acknowledge that you have informed the parents of the students who will be in attendance of the inherent risks associated with any field trip activities, and will provide executed Parent Permission Forms for each student prior to the field trip.
Chaperone Safety Policies
Groups who arrive without the required number of chaperones (a ratio of no more than 8 students to 1 adult) may be asked to leave. Teachers and chaperones are required to maintain control and discipline of their groups at all times. Please familiarize yourself with our courtyard rules on arrival and ensure all your group is aware of them. We ask that students are respectful of other users during their visit. Students must be accompanied at all times. Students must be accompanied by a chaperone while using indoor restroom facilities. Please advise us if you would like your group to shop in the store during your visit. No more than six children plus a chaperone are allowed in the store at one time. Please refrain from using your cell phone during your visit unless it is an emergency. Report any injuries to the front desk.
I have read and understand the above Chaperone Safety Policies for bringing a field trip to Twirl. By signing this agreement, I agree to abide by these policies and to assume responsibility for any damage done to Twirl property by my group. Twirl is not liable in case of personal injury through use of the Twirl facilities. Failure of all students and chaperones to comply with these rules may result in your group being asked to leave Twirl in addition to any other remedy for such violation as may be provided by law.
Please note, Twirl is a PEANUT-free campus.
