Site Preceptor Pathway interns complete the clinical and food service management rotations in three groups:

Group A interns complete these rotations from August to mid-December in Raleigh, NC
Group B interns complete these provided rotations from January to mid-April in Raleigh, NC
Group C interns complete these provided rotations from late-April to early-August in Raleigh, NC

This form is required as part of the Site Preceptor Pathway application process.
