Use this form to report any incidents that occur in Hackspace. This includes Health and Safety incidents, near misses, broken tools, and malfunctioning equipment. Please also remember to physically mark/cordon off broken equipment in the space so no one else comes to harm. This form and its contents can be seen by the board and the subgroup teams. Your contact information may be shared with the subgroup team to allow them to ask for more information if needed.
